Once you have decided to work with me on Nutrition I will send you the health and lifestyle questionnaire plus the food preference questionnaire. These will take approximately 20-30 minutes to complete and are submitted electronically. All personal data is encrypted.
I require at least 2 working days between from when I receive your forms to the Zoom consultation. This allows me the time to analyse your responses and draft any materials that may be relevant to your circumstances. Following the consultation, I will send you an email with all appropriate material which will have been agreed with you in our session.
Due to the work that takes place prior to the consultation and advice given within the consultation itself, 100% of payment is required at least 2 working days prior to the Zoom appointment. No Zoom appointment can be scheduled until payment is received.
24 hours’ notice must be given if you wish to cancel your appointment. All fees are non-refundable due to the work that is required beforehand to create your personalised programme.
Any failure to attend an appointment without prior notification may result in the consultation or follow up session being lost.
Maintaining regular contact with clients leads to greater success in achieving goals which is why I set a 3-month expiry period for all follow ups included in each package. Under special circumstances, extensions to the packages may be offered. Additional follow ups can be purchased where further support is required (minimum of 60 minutes at the cost of £60).
A Covid-19 form and Consultation form need to be completed prior to your first appointment. These are available to be downloaded directly from my website or can be completed on the day of the treatment.
Payments can be made on the day, either by card or cash. 24 hours notice is kindly requested if you need to cancel or reschedule your appointment.
